SEXUAL MISCONDUCT REPORT FORM

The optional form may be used to report incidents of sexual misconduct as defined below:

Sexual harassment
Sexual assault
Stalking
Dating violence
Domestic violence

This form is just one option. University policy prohibits retaliation against any person making a report.
